The Nursing Practice Act (NPA) is the body of California law that mandates the Board to set out the scope of practice and responsibilities for RNs. The Practice Act is located in the California Business and Professions Code starting with Section 2700. Regulations which specify the implementation of the law appear in the California Code of Regulations.

The Personal Identifying Information form (CDA 7006) is required by every applicant and/or provider as part of the complete application for initial and continued enrollment and certification as a Community-Based Adult Services (CBAS) Medi-Cal provider, and when facility and/or corporate changes occur.
Individuals required to complete this form include owners, management company/agency staff, board members, administrator, assistant administrator, program director, assistant program director, and ffice/business manager.

The Staffing/Services Arrangement (ADH 0006) provides information on current staff employed and services provided at the Center. Submit a CDA ADH 0006 to the CBAS Branch when professional staffing changes occur.
