Archstone Supported Learning Collaborative RFP Q&A
With the generous support of the Archstone Foundation, the University of Washington (UW) and the University of California, Davis (UCD), invite primary care clinics and community-based organizations (CBOs) to join a valuable learning collaborative to improve depression care for older adults and reduce mental health disparities. Through $20,000 one-year awards, clinics and CBOs will learn to partner and innovate to address patients’ medical, behavioral, and social needs to improve care. Partners will design and execute projects to improve depression care for older adults. Join California clinics and CBOs in an opportunity to collaborate and learn together. Best practices will be shared by UW and UCD experts and knowledge gained during the Archstone Foundation supported Care Partners project.
