 caregivers are the foundation of our nation’s system of long-term services
  and supports. Without the hard work and dedication of family members and 
 friends\, adults of all ages with long-term support needs would be unable 
 to live in the settings of their choice and remain engaged in their commun
 ities. \n\nToo often\, however\, family caregivers are unseen\, unheard an
 d unrecognized for the critical role they play in the lives of those for w
 hom they provide care. Many times\, family caregivers are not included in 
 critical care and support discussions when the responsibility for such oft
 en falls on their shoulders. \n\nTo better support family caregivers\, Con
 gress passed the Recognize\, Assist\, Include\, Support\, and Engage (RAIS
 E) Family Caregivers Act in 2018. The RAISE Act established the Family Car
 egiving Advisory Council which is tasked with creating the country’s first
  national Family Caregiver Strategy to address many of those issues and is
  helping to elevate the voices of family caregivers on the national stage.
  During this interactive webinar\, participants will hear on the Administr
 ation for Community Living’s efforts to implement the requirements of the 
 Act and provide an opportunity to discuss the role of adult day services p
 rograms as a key component of family caregiver support. \n\nObjectives of 
 this webinar include:\n•	Provide participants with an understanding of the 
 RAISE Act and progress to date on the Administration for Community Living’
 s efforts to implement its requirements\;\n•	Introduce participants to the 
 recommendations of the Family Caregiving Advisory Council\n•	Offer opportun
 ities to discuss the important role adult day services programs play in th
 e lives of family caregivers\n•	Hear from attendees their thoughts for spec
 ific actions adult day services programs can take to better support famili
 es and family caregivers.\n \nAbout the presenter: Greg T. Link. With more
  than 30 years of experience in aging and family caregiver support program
 s\, Greg oversees ACL’s efforts to implement a range of federally-supporte
 d programs and initiatives. His office has responsibility for the National
  Family Caregiver Support Program\, ACL’s dementia program portfolio\, the
  Lifespan Respite Program and other initiatives including the Eldercare Lo
 cator\, Community Care Corps and person-centered\, trauma-informed support
 ive services for Holocaust survivors and other older adults and family car
 egivers with histories of trauma. Greg holds a bachelor’s degree in psycho
 logy from the University of Central Florida and a master’s degree in gover

 iminated in the state of IN due to improper usage\, Independent Adult Day 
 Centers have pioneered new innovations in Healthcare Coordination in the a
 dult day setting that has demonstrated tremendous growth in the industry. 
 The COVID 19 pandemic exposed fragmentation in the healthcare system and t
 he need to monitor individuals’ progress for improved outcomes. Since late
  2020\, Healthcare Coordination usage has greatly expanded in the state of
  Indiana\, especially\, amongst waiver service providers. Just like curbsi
 de pickup\, this is a service that will stick around. \n\nWhat is Healthca
 re Coordination? Healthcare Coordination is a multifaceted program that cr
 eates a service individually customized to the unique needs of the person.
  It is a person-centered\, holistic approach\, which offers transportation
  to and from healthcare appointments along with a staff member who attends
  the healthcare visit with the individual. The program assists with medica
 tion management\, equipment allocation\, home assessments\, advanced care 
 planning and more. \n\nHealthcare Coordination has been an instrumental su
 pport to Case Managers and their clients in positively managing healthcare
  needs and outcomes. The webinar defines Healthcare Coordination\, the dif
 ferences between care management and healthcare coordination\, enhancing c
 ontinuity of care\, collaboration\, barriers that Caregiver’s face\, and c
 ase studies. Participants will understand the design\, structure\, and imp
 lementation so adult day centers can institute their own program. \n\nObje

 nity Development. Jessica graduated from Friends University in 2013 with a
  Master’s Degree in Healthcare Leadership. Jessica has worked in various n
 ursing/management roles including: charge nurse\, admissions\, staff devel
 opment coordinator\, and medical records coordinator.\n\nAngellica Jones c
 urrently serves as Assistant Director of Medical Services for Independent 
 Adult Day Centers. Angellica has been a Certified Nursing Assistant for si
 x years and graduated from Anderson University with an undergraduate in Li
 beral Arts and completed her education in three years. Angellica worked at
  Hoover Wood Nursing Facility from March 2015 to July 2018 and then was hi
 red as a Life Enrichment Professional at Independent Adult Day Centers\, J
